We are seeking a Community Nurse Liaison-Admissions Director to achieve the occupancy goals for the Community in compliance with federal, state, local, and corporate policies and procedures. Develop and oversee processes to achieve occupancy and admission goals for the Community

Responsibilities

  • Develop and implement the quarterly sales and marketing plan.
  • Work closely with prospective Residents, families and referral sources and close sales.
  • Develop and implement plans to increase referrals from current and new sources.
  • Obtain a working knowledge of the marketing and program development strategies of the medical community and other Long Term Care providers.
  • Assist in the development of new products and services based on referral source needs.
  • Provide discharge planners with timely assessments and answers to the acceptance of referrals.
  • Provide physicians and other key members of the medical community with information regarding the programs offered at the Community in order to gain referrals.
  • Provide the Nursing staff/Community with an accurate assessment (clinical and financial) of the potential Residents, including all documentation needed.
  • Collaborate with hospitals and physicians to accommodate Residents in the Community environment.
  • Identify and develop relationships with non-medical referral sources to obtain referrals for short term rehab and long term care.
  • Expectation is to be out of the building a majority of the time to obtain new business and develop referral sources.
  • Visit Residents during hospital stays.
  • Ensure quality customer service to the Residents, families, and staff the department serves.
  • Assume responsibility for department compliance with federal, state, local, and corporate regulations.
  • Demonstrate consistent management of resources.
  • Operate within the established budget guidelines.
  • Work with Bundled Payment Initiative Program.
  • Daily Compliance with CRM (Customer Relationship Management) tool. Input all sales activity daily.
  • Submit weekly activity summary from CRM.
  • Work with corporate office and Community team to develop strategies for marketing and initiate marketing material requests
  • Attend and participate in continuing education and in-service programs to keep abreast of changes in the profession.
  • Observe all Community policies and procedures, including but not limited to safety, infection control, Resident’s rights, and those contained in the employee handbook.
  • Maintain the confidentiality of all protected health information whether electronic, written, or oral exposed to either during the course of assigned duties or as a result of an incidental disclosure.
  • Provide support, as a backup, for the Independent and Assisted Living sales person (where applicable).
  • Other duties as assigned.

Qualifications

  • Senior Living or healthcare sales experienced preferred.
  • Bachelor’s degree preferred.
  • LPN or RN degree a plus.
  • Able to follow and give written and oral direction.
  • Ability to work independently.
  • Demonstrate dependability, cooperation, and interest in the care of the elderly.
  • Display leadership qualities, good communication skills, and a desire to continuously learn.
  • Possess the ability to communicate effectively and deal tactfully with personnel, Residents, families, visitors, government agencies, and the general public.
  • Pass background check before and during employment, as defined by Christian Horizons’ background check policy, with no disqualifications by committing or attempting to commit one or more of the offenses defined within.
