We are seeking a Chaplain to provide support to patients and their families on spiritual matters.

Responsibilities

  • Assists patient and family with spiritual concerns associated with the dying process by utilizing spiritual care assessments, life review counseling, etc.
  • Conduct and/or attend religious services for patients, as required and requested, and be on-call as necessary.
  • Meets the spiritual needs of all patients and their family/caregiver per the plan of Care and informs patients and their families of the spiritual care services available.
  • Serves as a spiritual advisor and counselor to hospice staff and volunteers
  • Regularly consult with other SHH employees regarding the status of patient interests, energy, general health, and spiritual needs.
  • Foster self-respect and a feeling of worth in each patient by consistent kindness, understanding, and patience in their care.
  • Function as part of an interdisciplinary team to provide quality care to all patients.
  • Assume responsibility for department compliance with federal, state, local, and corporate regulations.
  • Maintain up-to-date, accurate, and appropriate documentation daily.
  • Attend and participate in continuing education programs and staff meetings.
  • Observe all SHH policies and procedures, including but not limited to safety, infection control, patient rights, and those contained in the employee handbook.
  • Use standard precautions in the care of all hospice patients, regardless of diagnosis or presumed infection status.
  • Maintain the confidentiality of all protected health information whether electronic, written, or oral exposed to either during the course of assigned duties or as a result of an incidental disclosure.
  • Assist in developing and coordinating positive relationships between Hospice and community religious/spiritual resources.
  • Serve as a liaison with community and local religious leaders to access their resources to help meet the needs of Hospice patients/families. Initiates contact with patient’s spiritual/religious community.
  • Participates in company and community programs as requested to promote professional growth and understanding of Hospice care, spiritual care and grief and loss issues.
  • Participate in Safe Haven Hospice's Quality Improvement program.
  • Provide spiritual care services and bereavement services in accordance with the Interdisciplinary Plan of Care.
  • Assists the members of the Hospice Interdisciplinary Team to understand the significance of spiritual and emotional factors related to the patient’s care and family beginning at the time of admission and continuing after a patient’s death up to thirteen (13) months.

Qualifications

  • Bachelor’s degree required.
  • Previous chaplain or other related experience preferred.
  • Member in good standing of the Christian Church.
  • Able to follow written and oral directions.
  • Ability to work independently.
  • Possess the ability to communicate effectively and deal tactfully with personnel, patients, families, visitors, government agencies, and the general public.
  • Display leadership qualities, good communication skills, and a desire to continuously learn.
  • Pass background check before and during employment, as defined by Christian Horizons background check policy, with no disqualifications by committing or attempting to commit one or more of the offenses defined within.
  • Valid driver’s license and daily access to a reliable and insured vehicle.
  • Ability to exercise judgement and discretion in confidential matters.
