We are seeking a Certified Nursing Assistant to join our team at Safe Haven Hospice.

Responsibilities

  • Provide personal care to the patient in a compassionate and understanding manner consistent with the hospice philosophy. Listen to all customers and respond timely to their needs.
  • Display a positive and professional image and attitude in all relationships with patients, families, staff, and communities
  • Review and follow plan of care for each patient every visit.
  • Report any change of condition in patients immediately to the case manager.
  • Responsible for maintenance of own entries in to paper and electronic medical records, assuring accuracy, completeness, and compliance with regulations, certification standards, legal, and ethical standards.
  • Use proper equipment and techniques to ensure the safety of patients.
  • Assist patients to maintain good personal hygiene and assist with bathing, and care of mouth, skin, hair, and nails in accordance with the plan of care.
  • Provide bed bath, shower, or tub bath in accordance with the plan of care.
  • Assist patient with use of bedpan, urinal, commode and bathroom.
  • Assist with routine catheter care and enemas according to the plan of care.
  • Assist with feeding and dressing patients in accordance with the plan of care.
  • Change dressings as necessary.
  • Assist patient in and out of bed with ambulation.
  • Monitor and record patient temperature, pulse, respirations, and blood pressure as directed in plan of care.
  • Provide appropriate body alignment, positioning, and range of motion exercises as necessary.
  • Provide holistic, family-centered care across various treatment settings to improve the quality of life.
  • Follow all written and oral instructions as given by supervisor.
  • Maintain the confidentiality of all information pertaining to patients, families, and staff and adhere to all HIPAA rules.
  • Participate in training, staff meetings, and in-service education.

Qualifications

  • Successful completion of a state approved nursing assistant training program.
  • Current and active certification on the Health Care Worker Registry through IDPH.
  • Able to read and comprehend written patient care instructions and chart patient conditions.
  • Ability to work independently.
  • Demonstrate dependability, cooperation, and interest in the care of the patients.
  • Possess the ability of interpersonal skills and communicate effectively and deal tactfully with staff, patients, and families.
  • Valid driver’s license and daily access to a reliable and insured vehicle.
  • Previous experience working in hospice, home health, or long-term care preferred.
  • Efficiency and a working knowledge of computers.
